Private Practice Partners
The Physician Foundation strives to continue the private practice model of medicine in San Francisco. Private practice physicians refer patients needing specialty care to our members. In turn, our members refer their specialty care patients to private practice physicians.

Health Plan Partners
As part of our on-going commitment to facilitating the best public health practices for our members, we have developed health plan partnerships with other medical organizations that meet our high standards.

Community Partners
Through our partnership with the the San Francisco Clinic Consortium, we have developed a charity care program designed to provide primary care to low income and under-insured individuals in San Francisco.
PFCPMC has been an active participant in San Francisco's Project Homeless Connect and has worked hard to improve the health of the Bayview Hunters Point community with participation in community events on health issues.
